Taking The Next Steps
To learn more about Horizons Counseling, call (309) 557-1124. An intake counselor will talk with you about why you are seeking counseling and will obtain basic information about you. After that initial phone call, the following will occur:
1. You will receive a packet of information in the mail. Please read the information carefully, fill out the requested forms, and return them with a copy of the front and back of your insurance card to The Baby Fold's Family Center at 612 Oglesby Ave, Normal IL, 61761.
2. A counselor will be assigned to you and will contact you to schedule
your first appointment. Your counselor will be able to meet with you
within one week of your initial inquiry in most cases. You will find our offices to
be private and comfortable.
3. Your first appointment will last about 90 minutes. Please bring your
copay with you. Your counselor will
conduct an assessment with you during the first appointment that will
lead to development of a treatment plan.
4. You and your counselor will identify treatment goals.
5. You and your counselor will schedule appointments at your convenience.
6. Every six months you and your counselor will review your treatment
plan.
7. When you are satisfied with the progress you have made, you and your
counselor will plan for termination of counseling services and will develop
an aftercare plan.
What about billing?
1. You will mail a copy of your insurance card with your intake packet prior to the first session.
2. Horizons Counseling will submit the initial billing to your insurance company for confirmation of your deductible and co-payment requirements after your first session.
3. After we confirm with you the amount that you are responsible for paying, you will need to bring that amount to each session. You may pay by cash or check and your counselor will provide you with a receipt. Your portion of the payment is due at each session unless you have made other arrangements with the Horizons Counseling billing office.
